Dental
You may select either the DMO or the PPO for dental coverage.
Please note that once you have selected either the DMO or PPO, you will not be able to change your dental plan election until the next annual enrollment period (unless you have a qualified change in status).
Dental Maintenance Organization (DMO)
This plan offers unlimited benefits, with no deductible, and 100 percent coinsurance for preventive and basic services and 60 percent coinsurance for restorative services.
Orthodontia benefits, payable at 60 percent of the charges for one course of treatment per lifetime, are available to covered members regardless of age with no lifetime maximum.
You must choose a Primary Care Dentist who is responsible for performing your basic dental care and managing all your dental care services, including referring you to specialists when necessary.
Dental PPO
This plan offers benefits limited to $2,000/year with a $50 annual deductible ($100 per family) and coinsurance (the plan pays 50 to 80 percent of the cost of most services).
Orthodontia benefits are available to covered members regardless of age, up to a lifetime maximum amount of $1,500 (separate from the annual maximum). You do not need to choose a Primary Care Dentist.
